It might be easier for you to print this form and have it filled out prior to your first appointment with us. If this is not convenient for you please don’t worry, we have copies and can fill out this form with you in the office.
For your convenience, we have listed some helpful thoughts below. This will help to make your first visit with us easy and efficient:
- Call us at 856-467-0004. Speak with one of our receptionists and make your first appointment. You may also request an appointment online at the top, right hand corner of this page. We will be happy to call you back to confirm a day and time.
- Have your pet’s medical records faxed to our office prior to your appointment. Our fax number is 856-467-9549
- Be sure to write down all the medications your pet is taking
- Be sure to write down all medical concerns and conditions you are aware of
It’s best to write down notes about your concerns or any questions you have prior to your appointment. This insures everything important to you is covered to your satisfaction.
